Baseline Questionnaire (Participant)

This questionnaire is for people who have used Pre-Exposure prophylaxis (PrEP) in the past and who have recently been diagnosed with HIV. This means having been diagnosed in the last 3 months. We want to better understand how people used PrEP in the past, and whether there are people who acquired HIV despite using PrEP.

This study is taking place in more than 80 clinics around the world, and is being run by Centre of Excellence for Health, Immunity and Infections (CHIP) and University College London.

The information you provide is completely confidential. Your answers will not be seen by the doctors and nurses in the clinic.
